I was playing as shiv, started farming then when I used dash I went straight through the wall and fell out of the map. Unstick, F7 kill didn't work. The way I got out was because I could use zipline under the map, so I spammed it and when someone came close I used killing blow. I managed to replicate it in a sandbox game.
