Seems like Rem can get OOB without any jumps or dashes.
To replicate, approach a longer crawlspace and attempt to exit it. Intermittently crouch and quickly release while holding forward. Rem will kind of clip against the entrance, and sometimes zoom straight through it. If you uncrouch right as he is beginning to zip through, you will OOB.
I believe this is due to some logic attempting to move him through the crawlspace. Crouching and beginning to enter but quickly uncrouching keeps him from going through, but sometimes you can uncrouch and still hit whatever trigger exists that makes/allows him to enter the crawlspace. This would clip him with the ceiling, so the game moves him out of bounds since there is no other space.
I'm finding this method is fairly consistent compared to ones involving jumps.
