This is a known bug that happens to me almost every game. Supposedly it happens when you die while casting an ability but I haven't been able to verify that (also I don't think MikaelS was casting anything when he died there). Since it doesn't happen in the test range for me I feel like it might have something to do with latency.
Had this recently as well with Bebop and Seven

For Bebop it was casting his Ult before being anviled/grabbed by Mo n krill , as for Seven it was Ulting after and then grabbing zipline.
Wouldn't stop until i died, checked replay between 13-20mins and it didn't show.
Take it's either client side issue with animations/animgraph breaking when too much is happening?
The hero animations sometimes break on respawn. The model starts to glitch out or something. It doesn't fix itself on another respawn or disconnecting and reconnecting.

I've had this issue with most heroes I've played since the shop rework update at some point, it's not very consistent and it always is resolved upon dying and re-spawning again, I'm not sure if it has something to do with how you die or holding down buttons while dead then re-spawning
To clarify I've had this happen with Pocket, Abrams, Calico, and Ivy. That's just the ones I can remember off the top of my head.
