HUD does not show which teammates are using VOIP

My game does not show which of my teammates are talking (i.e. it does not show the speaker icon underneath their hero avatars); making it very difficult to figure out what's going on.

I have a 34" widescreen monitor and have wondered if that might be the cause.

This issue also occurs when playing on high ping (e.g. 150 ping).

Oh, that might be it then, thanks. I play on 168 ping.
