Upon being ulted by Lash while simultaneously casting my ultimate, I entered my ultimate but the cooldown was not used. It was purely visual and I got lifted, slammed, and died while clouded in my black smoke. After respawning, the ultimate effect lingered around my model as shown:
This visual bug persisted even when I used gloom bombs, leaping slash, and Ava, but it went away after I used my ultimate again.
Match ID 39014852 and it happened when I died on Green Walker.
Additionally, Leaping Slash's third upgrade states that it reduces the cooldown by 50% on hero hit, but it was reducing the cooldown on neutral enemies and mid boss as well.
