Using Refresher during ultimate (when he's holding the stakes) will refresh your abilities EXCEPT your ultimate (you don't get another cast). Meaning, you can fat finger the refresher, when all your abilities are up, during stake form, and you go on cooldown on ultimate - meaning you used refresher as if you didn't use any abilities (which should forbid you, which the game already does).
Considering he's a triple wielder, that means refresher (for interesting synergy) either gives him 3 more shots, or the ultimate comes back, fully ready after the first 3 stakes were shot (or timed out).
