Mostly happens to me on shiv. If i am killed during the animation of something active(slice and dice, punch, item activation, etc.) i am stuck in the bugged startup of the animation when i respawn. Performing the action again usually fixes it however sometimes with the additional bug of extreme muzzle flashes and a general hit to game performance.
