Happened on the final selection phase. I (Paradox) was full items, had enhanced Bullet Lifesteal, and slected Enhanced Leech with a bonus Opening Rounds Attached. When I selected Enhanced Leech + Bonus Opening Rounds, I only acquired the Opening Rounds.
I assume it is because Bullet Lifesteal builds into Leech and something messed up while "building" Leech and buying the bonus item. The bonus item was put into my inventory first and then Leech/Bullet Lifesteal disappeared since inventory was full.
