Probably been reported a ton since its a pretty common bug I come across but might as well report it. If you have a soul bag and you die in the teleporter the bag will not be reachable. the icon will be on the map until it despawns. Don't have any replays of it since I only thought to report now but it happens frequently enough that I'll add a match ID and video when it occurs again.
Soul bags are impossible to claim if the holder dies in the teleporter
