Supposing this item needs a buff, here's an additional stat:

When user heals above 1000 health within a short window, Cooldown Reduction increases to 33% for the remainder of passive duration.
When the user deals beyond 1000 damage within a short window, Debuff Resistence increases to 33% for the remainder of passive duration.

You can substitute the 1000 threshold with a smaller number or alternatively a percent based threshold relative to the target of healing/damage if you want the item to scale better across early game and late game.

The main thing I would say is that the 'when healing' condition needs to be clarified—what kind of healing does it include? Technically, extra regeneration counts as healing, so if you have extra regen from lane and this item, you'd essentially have cheap, permanent cooldown reduction and now a beneficial debuff reducer-like effect during fights.
