Match 62713 - unfortunately I couldn't get the exact time due to a bug downloading replays, but it was in the last ~10 mins of the game after at least a couple spawns.
The mini boss which is usually in the basement of the center area spawned (or got stuck at some point?) in the upstairs area (where the urn is usually deposited) with his head stuck in the ceiling. He could not be damaged or killed.
