At around 34–35 minutes mark, my fourth item slot became permanently red and unusable, so I had to move the item (echo shard) to another slot and remap my key bindings in order to play "normal" (since I did not have all 4 item slot filled). The issue persisted till the end of the match.

No, I haven't, since I had a free slot in every case, and changing settings is quicker than other variants. Maybe I'll try the next time it happens, just for the test. It should probably help, there are no in-game mechanics that prevent you from using a specific item slot, so I think it's a client bug that should be "fixable" by restarting the game.

So yeah, I've got in one of those situations today again, and can confirm that leaving and rejoining fixes it.
