I HAVE REPRO STEPS: spectate someone who has the same active item as you. if they use it such that it's still on cooldown when you respawn, then the active item slot state continues to reflect _their_ active item state rather than _your_ active item state.
i was spectating the seven (who used fleetfoot a couple seconds before i respawned. i respawn and my fleetfoot icon still visually shows being on cooldown; i go down lane and pop fleetfoot and the visual cooldown state stayed bugged. the lil error popup with specific timer was correct, though.
you can fix this by swapping the slot it's in but that's annoying to do mid combat
