It only works the first time you use debuff remover in a game and won't work after you die or sell/rebuy the item.
It's only supposed to heal you and give move-speed if you remove a debuff, so this is a bug.
It's also worth noting that Debuff Remover isn't removing any invisible debuffs or anything. If you use Divine Barrier first and then use Debuff Remover (with no active debuffs), you still get the cleanse bonuses on the first time you use it.
