Cool seven visual bug

I was playing seven and I believe I ulted as I died, making me the ult skeleton for the rest of the game. (I went into practice tool and could not recreate it)
PLEASE give me a way to recreate this it was so epic! Halloween day perhaps? Spooky Seven?

I also had this issue, it was only locally and ended when Ulting again. From memory I think I went to cast ult and died before it registered.
