Apollo's ultimate ability slipped under the map.

Came here to post the same thing, I'm able to semi-reliably get him to clip out of bounds if his ult would end near the tunnels underground.

I'd guess it's something like 'the ult looks for a place to place him when the animation ends, and if it finds land near a certain space it'll drop him off there'?
 
I've been able to reliably recreate this bug in the lobby as well:

Doesn't look like a line of sight check is being done or weird collision layers.

Also got stuck inside the Rem/Calico tunnel as well. Once where it took me there immediately, and another time where I slipped under the map and it teleported me back to the closest point back into the map, which was also the tunnel. Should probably make it so that if a hero does go under the map, they aren't teleported to the tunnels

Had the exact same thing happen to me.
His 3 does it the most, usually around stairs and corners of walls and along walls.

This mainly happened around the opposing patron, mainly the Arch Mother right side.
