Found while playing, and managed to replicate it once in the same game. If Yamato gets stunned while tethering to a target, the grapple will remain between her and the target until the target dies. I believe I was shoulder-charged by Abrahms once and stunned by IVY another time. Other stuns on the team included Grey Talon's bird, Infernus Ult, Mirage Knockup, and I believe Vindicta was on the enemy team as well.
Enemy team also confirmed that it happened on their side when it occurred
