Wraith's teleport and ultimate are both abilities with different cast options but both are set to ability default whenever you relaunch the game and I haven't experienced this for any other hero but Wraith. I'm not sure if it's a client-side bug or not but I pick quick cast every time I log in and it still gets reset to ability default (confirm cast).
