If I have four items with instant cast and hit ZXCV all at once, usually only a subset of them activates.
However, while they're all on cooldown, if I hit ZXCV all at once, it does show the red shake animation on all four of them at once, so it's not my keyboard.
E.g. Vampiric Burst + Healing Nova + Heroic Aura + Split Shot
