Unable to self-cast active items with "mousewheel"

I self-cast active items like healing rite/ echo shard with "mousewheelup" and use "Alt" as my alternative cast button. I am unable to self cast when the button is set to "alt" however using other keybinds seems to work okay

Came to report the same. Tried using all item cast options. Items that don't have alt cast function work properly with MWheelUP/Down such as Alchemical Fire, Warp Stone, Metal Skin, etc.

Have the same problem still, "alt" for self cast with "mousewheel" works if combined with ctrl or shift, but not by itself. Please fix
