item that passively reduces the amount of lifesteal that damaging you provides. i envision this item on a tankyer build to provide a counter to ppl that you cant kill because they have so much life steal. this item could be limited to the early game by having it remove a fixed amount of health that will end up being less significant in the late game. If it instead was a percentage reduction it could remain relevant later into the game. It could also affect how quickly the mid boss yellow bar refills to help allow for taking it quicker.
 
There are a lot of antiheal in the game, problem is only that its hard to fit in your build if you are not infernus of haze. Not to mention this item buffing one class and does not give anything to others, which is disbalance. Midboss is already dying within 10 secs if your team is not full of tanks whose building spirit damage.

for the boss part i was imagining it would make the midboss quicker with less ppl, more for sneaking while your team is split up or when your in a game where you are having issues getting the ppl to group for midboss.

i think it is okay for an item to favor one class and there are lots of examples of that in the current item set. if we are using that logic items like melee could be argued problematic since they are better/easier to use on close quarters heroes.
