Shrine "too far" in weird circumstances.

I sometimes get "too far" (and reduced damage) on Shrines in situations where it certainly shouldn't happen.
The two recent cases were:
Lady Geist's bomb thrown either extremely close to the shrine, or literally at the shrine. I'm not sure if the initial explosion was bugged, but the ticks from the third upgrade were getting "too far" and extremely low damage. If I threw it a bit further it dealt full damage to the shrine.
Lady Geist's Life Drain. Here it seemed kinda random if it would get "too far" or not.
