I dont know whether it would be weapon, health, spirit. It could be a 6.4k item and then the user can unlocked a 5th active item slot. What do you guys think? So technically it would cost 12.8k to buy another active item. Would be good opportunity cost.
There is issue of key mapping as well as balance and econ. This will make it a bait item, and towards late game would be straight up oppressive.
Why is it assuming that the active will be tier 4? Continuing on, assuming you can get any active item (no duplicates)

You can get Return Fire, Metal Skin, Colossus, Warp Stone, Etheral Shift

Also consider you can get transcendant cooldown. We are looking at a lot of actives being up at once, effectively pushing dominance of buying actives with transcendant cooldown to rush. That would dominate the meta and would not be healthy.
It would be bait for lower elo and higher elo it would be too dominant and a bit unfun in terms of itemisation, as it sucks out the impact on decisions to be made. Would strongly not recommend.
