penalty cooldown for leaving the match

why does it say when you quit a match youll recieve a cooldown penalty but you dont, my friend rage quitted 2 times and havent recieved a penalty, is it a third or like fourth time thing? or is it a bug?

It's issued after the 2nd match ends from what I've seen.

I wouldn't test it but valve typically enforces these penalties on repeated early quitting like 3 or more. Likely not a bug
