If mousewheel scroll, either up or down, is bound as melee, melee animation is not played when using that keybinding. Melee itself works normally, both damage and melee sfx/vfx are triggered. This behaviour seems to be unaffected by the hero you choose though I have no idea if this is local client only or if other players will see this as well.
In the video below, I do first couple of melee attacks using normal keybind and then switch to using mousewheel scroll.
