I used to be able to press ult then sanguine retreat while ulting.
Ive tried this recently again and for some reason I cannot use sanguine retreat while in ult.
I changed key binds for sanguine retreat as my 2cnbd ability is on my mouse but doesnt seem to change anything.
However If I use sanguine retreat first and then ult, I can use my second sanguine retreat while ulting.
EDIT: this seems to be only an issue related to me as I was trio qued and in the hideout both of my team mates had no issue starting the ult then using both sanguine retreats during the entire ult duration.
I tried this in my trio/duo and solo lobby and I have the same issue no matter what.
