The bug often happens when the target in front of me is moving. This is the first game where I have been able to record it. Match Id: 13283167 @ 29:10.
It doesn't show anything in the replay, but in the clip I posted the character clearly moves and the ability temporarily goes on cooldown, realizes it didn't actually cast the ability, and then returns the spent charge, and moves Lash back to the ground.
This might be server desync. But it didn't happen at all before the 8/29 patch which removed the Stamina gain from Grapple. I have no idea why this change caused it or if it is just I never ran into the problem before then.
