Before we get Ivy ult nerfed/ changed. I think its important to adress two things.
1. that sometimes the ultimate becomes invisible. Both Ivy and other players can't see it. Can't replicate in testing but happens often in online match, Theres many youtube 1 videos 2 with ivys 3 in them that you can't see when an ivy drops her bomb. (Skipping to every single time Ivy uses her ult. you won't see the visual), It only shows when Spectating 4 or watching a replay

2. Hurt box aoe is inconsistent with the visual aoe. They spread at different speed, Easy to replicate in testing alone. private testing video.
