Idea for a Melee item: Double Edge Punch

An idea of a melee build focused item intended for Burst/Initiation but not sustained fighting.
Not quite sure on the numbers, but the concept is an item that greatly increases the damage of your next melee attack, at the cost of Melee attacks now dealing self damage.
Something like this:

15 Second CD
+100 HP
Your next melee attack has:
+120% Melee damage
+25% Area of effect
-All your melee attacks now deal 50% of their damage back to you.

Again, not sure on the numbers, but could be a fun idea.
