Grey Talon crashing to desktop

Not entirely sure why but whenever I play Grey Talon I've been crashing to desktop pretty frequently. This has only happened when playing him.

Something simillar has been happening to me as well, it only happens when I'm ulting and about to hit the ground.

it's happened to me randomly. once i was just climbing a wall in jungle and the other i just let go of a charged shot at paige
