I was walking around the hideout when I suddenly fell through the map while in rems bedroom, with some trial and error I was able to consistently recreate it by standing on this pillow and jumping (how you know you're in the right spot is that the vent will hardly knock you back at all when you jump) then heavy attack. Attached is a picture of roughly the right spot you need to stand, a picture of what will happen if you are near the right spot (you get stuck behind the furniture and the only way I could find to get out of this was the kill command), and a picture of me through the map.
