Can't use certain abilities after death (Possibly related to non-default keybinds)

Has happened to me every other game, when I explained the reason for pausing to rejoin my teammates were familiar with the issue, which makes me believe whatever i've done with my client is not causing this. So this is a bug.
As I am not sure what exactly causes this, i can only attach the matchID and timestamp since my last use of the ability - after that i couldn't use it until i rejoined the game

happened again
40469219 ~7:05 last usage of drifter's 2 before it bugs out, try to use it again in the next minute and i cant, also noticed in replay it got used by itself right as i paused this time.
I didn't click it, it physically couldn't have happened because i get my right hand off the mouse (my drifter 2 is bound to RMB) to click p to pause. <- that didn't happen in the previous time i noticed the bug though.
So far the only pattern i can notice: 1. I miss stalker's mark 2. die while its still on cooldown 3. after i respawn i can't use it even if its not greyed out because of being on cooldown or anything
ive tried replicating it in sandbox but no success

Apparently its not special to drifter and has been going on for a while. see many threads citing to having this problem for over a week ago (same experience for me, but I thought I was crazy):

This bug has now been in the game for at least a week and hasn't been fixed. It's happened more tiem than i can count at this point, but this is the most it's happened to me yet. In this match (Match ID 40265023), I die at 11:50 to drifter, and then I go in to farm a camp when my teammate is getting chased. They then turn to me, if I am able to use my stake, i possibly live, possibly not, but there wasn't even a chance because i just couldn't use an ability in a game where abilities are key. Thread 'New Characters unable to to use certain abilities on death.'

After using certain abilities on the new characters like Rake on Mina, when I die I am unable to use the ability when I click the keybind. Instead I have to parry or use another ability like my Sanguine Retreat (which will auto activate disabled ability).

So far this has happened on Mina (Rake) and Billy (Blasted). Both of these abilities are bound to different keys and changing keybind did to fix the issue (including resetting them to default). The only guaranteed solution was to quick the match and reconnect/start a new match.

in match 40080856 i experienced the bug that disable abilities. i have no idea what triggers this bug, but at seemingly random moments throughout the match i was unable to use certain abilities. the first instance was during laning phase, i was unable to use bebobs primary or secondary ability. later in the match the exact same situation occured. then i was able to use the secondary but not the primary. this happened twice, finally i was unable to use the primary or the third ability. each time this happened, the only way to fix it was by restarting my game. death did not fix it, nor did...

26 minute mark and on, i am entirely unable to use flog at first and then grapple after a respawn.

Some commons:
1. Leaving the match and rejoining fixes it. (some say resetting keybinds to default helped them, some say it didnt)
2. Dying somehow triggers it
3. (This is an assumption based on people who did mention it) they have different keybinds from the default

it happens almost every match for me on drifter, sure, I know the solution but I can't know when it happens

here, got the bastard on tape, since in replay it looks like chicanery, and i am NOT crazy.
on the demo I am pressing his 1st ability - Rend. i know my keybinds are odd (which might be the reason for the bug), i might be autistic
this also happened after a death (it was a rejuv death this time)
you can see the keybind repeatedly twerking as I am mashing it, but nothing is happening
