Unable to use active items in slots 2 and 4, and unable to use voice chat.

I am unable to use the active items in slots 2 and 4. This issue has been occurring for the past two to three weeks. I have tried changing keybinds, playing on a new account, reinstalling the game, verifying the game files, testing a different keyboard, and even playing on a different motherboard and CPU, but the issue persists. The active items usually work for the first 5–10 minutes of a match and then stop working.


I am also experiencing an issue with voice chat. It only works in some matches, but in the majority of matches it does not work at all. When this happens, the voice chat icon indicating that I am speaking does not appear, and other players cannot hear me.
