When dead and spectating a player, a red selection diamond can appear (which usually appears when a player is targeting an enemy with an item), and doesn't go away until you respawn. Interestingly this only appeared when I spectated Infernus, not Pocket, who was the actual person with the item (Decay), and was also dead at the time. This also doesn't show up in the replay.
