Stuck in walls and could not get out for the rest of the match

I was playing with Lady Geist and got Phantom Strike. In the last 10 minutes of the match I blinked to someone and got stuck in the walls. This happened before but I was always able to blink out. This time I couldn't and it cost us the match. Please fix this issue, it happens way too often with Phantom Strike,

While this is clearly a bug, if you have a Dynamo on your team you can have them get close to the wall and warp you out (I've done it before).

This happened once with a Mo and Krill where they got stuck inside of the vent near the urn spawn area while using Phantom Strike. They tried phantom striking an enemy near/behind the Vent and the game accidentally placed them inside/clipping into the vent and then they got pushed inside of it instead of out. They could see outside the vent since no textures inside but they could not shoot or be shot. iirc only way they got out was by getting AOE killed by the enemy team after asking for help lol. Happened ages ago so no match ID.
