When I bought the refresher, I had to remove another activate item, I removed the healing rite, but instead of getting the refresher as an item to activate, I got return fire even though I didn't have this item even before. so in the end I chose the refresher and couldn't use it. I suspect that the game in compensation for the healing rite gave me a return fire, so there was no slot for the refresher I wanted.
