Steam Rich Presence not matching in-game status

In-game people always show as at the main menu. The steam friends list shows them in-game and what hero they are playing and how much time has elapsed.

Expected behavior: in-game should match steam friends information. I am on a separate server region from some of my friends, but even after switching to their server, the information is still showing "main menu" only.

That's weird. It shows when my friend is in-game and what level they are.

Yes it shows me in-game as well for someone on a different server (They're Aussie and I'm American). However I haven't been able to see since they haven't played in a few days but before that I was able to see their activity and spectate their game.

Yep, I can spectate their game just fine. But their status shows as in Main Menu anyway.
