Simple item idea "Rebirth Homunculus"

Pretty self explanatory, there have been attempts in the past to make a "Soul Rebirth" type item, and now that the rejuvenator has been changed, why not re-introduce it as an item?

I also thought maybe like glass cannon it could reduce max hp, but I'm not sure if it's necessary.
