Shiv dies from Pocket ult with Bloodletting

If Shiv is ulted by Pocket and then takes any other damage, for instance a gunshot from Pocket, then Shiv will die from the deferred damage. Any other hero would take the gun damage instantly and then not die from the Pocket ult but Shiv defers both damage sources causing him to die. Assuming this isn't intentional Shiv should take damage that can kill him first or take the deferred damage in order of when it was applied.

Affliction itself cannot kill, but can bring you to 1hp.
If Shiv gets low enough that the deferred damage, even 1hp worth, will go off while affliction holds them there, that 1hp is enough to tip the scale.
