Came up with an idea for an item that not only counters insta-kill heroes like Vindicta and Shiv, but also serves as a replacement for Soul Rebirth.

Saving Grace
+10% ability range
+2m/s sprint speed

PASSIVE (260 second cooldown): When a killing blow is received (any attack that reduces your HP to 0), your HP is instead set to 1, and you become Invincible, and are immune to negative status effects for 3 seconds. You are disarmed for the duration of this buff. You also receive +3m/s move speed for 5 seconds, which decreases over time after the buffs duration.



If this seems too overpowered, perhaps it might be best to make Ethereal Shift a component for this item.
 
Soul rebirth was removed for a reason. This seems to be even more broken.
I can see how the item can seem broken, but I thought about how it might play out in practice. Tried to make it so that the user has a chance to get out of a bad situation, but give the enemies enough of a chance to counter it when it procs. I also gave it a long cooldown like Refresher. If I were to nerf it, I'd change the buffs to being invulnerable and unable to be stunned or movement locked, but can still be movement slowed, and maybe lower the move speed buff, as well as slightly lower the time the buff is active.
 
So, we're just giving the old Yamato ultimate to anyone who uses abilities now? Sure, you're disarmed, but I can't wait to see something like a Shiv ult clean up a fight while being immune to crowd control and still having increased move speed.
