Multiple crashes on loading in and unstuck crashed my game

loaded into the game I opened the builds menu and instantly crashed reloaded back in to be stuck in the starting area alone and when I hit unstuck it sent me to the left most teleporter on the map which was not open so I tried to suicide and my game crashed and gave me some vpk error that I did not get to screenshot and could not even launch the game so I had to reinstall it

My PC has been crashing alot lately when trying to play this game, as soon as i load into the lineup lobby, my monitors go to No Input but i can still hear the sound but not normal where you can hear people talking, just sound. i have to reboot my PC, Resume match, same thing about 3-4 times in a row. I eventually give up and get banned for the normal 15 mins or so. Today i was able to play about 30 mins before the monitors cut out.

update.. happens sometimes when joining the hero sandbox as well.
