Had this occur on a recent game.
I was making my way to mid to try and do something about the rejuv and the graphic stayed behind despite having been claimed already, which confused the ever living crap out of me, making me waste time and effort. This escalated into a lost game eventually. I wasn't the only one who had it happen on our team when I spoke about it, but the enemy team didn't seem to have the issue. Matchmaking and thus ping related? I don't know.
