Melee Lifesteal Inconsistent

I was farming and hard-hitting the NPCs and sometimes it would automatically reload whilst other times it wouldn't. Haven't seen this issue happen in the past until today.

I presume you are talking about Melee Charge, as that is the item that reloads on melee.
It was changed last update and has a cooldown now:
