This happened in one of my games while I was chasing someone and I was very confused because I thought I had pulled out my ult but in actuality it got cancelled by me mantling despite it playing a voiceline indicating that I had indeed pressed 4. When I asked other players if they heard my ult activate they told me yes. Here is a clip demonstrating it in the hideout.
