I think a good balance change to shiv would maybe to be something in the direction of splitting his rage in to phases, so the more you fight the stronger you become. Make it so it wears down pretty fast when he doesn't fight, but comes up fairly quickly. This will make him play a bit more like a close quarters Lady Geist. She also has a damage amp but directly to one or more targets. These stages could give additional bonuses also, maybe give first phase a bonus to NPCs to help him with jungle and some laning and last stage his abilities get modified like usual.
I think it could make him a bit smoother to play also, not having the feel to constantly clear jungle to maintain your rage instead you know you will get at least some amp after a bit of poke, maybe some gun damage and a knife and you have your have your first set of 5% dmg bonus, a bit more and you have 10% and so on until he is full of rage. If you add some visibility of which stage he is in, he will make a more fair opponent in one sense, gives more of a warning instead of being a on/off switch where he suddenly gains 25% dmg and double dashes you.
Hopefully this would also mean no more having 0 rage and all of jungle is clear and no good lanes and team fight is starting up.
