probably happens with other characters but kelvin was more common for me.
my +attack and -attack is binded to mwheelup/mwheeldown, spamming mouse wheel may not trigger the cast (or rather, atleast execute the ability UI icon but not the ability in the world.) this obviously can be avoided by not spamming attack, but it could maybe potentially happen with a normal attack of m1. This report also may eventually help someone that had a similar strange problem.
