The idea is that it has the same toggle as Blood Tribute but active is: replace the Vitality health bonus with a HP regen bonus.
It would sort of work like Fortitude but it has the down side of lowering your heath when active. Just give it an ok cooldown so you can't spam switching between the two. Maybe have a multiplier to HP regen for fun and a negitive % HP down when active. And during the switch getting damaged stops the active HP to match the current % of HP when Max HP is going back to normal Vitality health bonus levels.
