Not sure if its specific to items, but I often see it in Fleetfoot and rarely in Slowing Hex. I can activate the item, which works perfectly fine, but it does not show the cooldown remaining on the item. So I have no idea when it comes back up or how much time I have remaining.
The fix I've seen is to drag the item to a new slot, but that is a really crappy fix..
