Steps to reproduce:
1. fill all spirit slots
2. attempt to buy Mystic Reverb
3. select item to sell when prompted
4. press escape instead of selecting ability
5. you now have Mystic Reverb but it will never trigger on any ability
This happens to me almost every day and I usually notice it too late to sell it for full price =(
