Time and place: Around 11:20 blue lane walker near Vindicta
So one of the Creeps landed on top of the walker and got stuck in the air somehow also becoming invincible. He was invincible to players and other creeps, therefore creating a creep traffic jam. This would have been a bigger issue if not everyone leaving this match. I have seen this multiple times already in different matches.
